A small team of clinicians have worked to embed RESTORE2 within care homes, and in those homes trained it has been a huge success. We have heard how it has enabled early identification of need, improved communication between care homes and primary care, empowering care home staff to escalate their concerns and to be heard.
However, the pilot experienced significant challenges during the course, including the impact of the Covid-19 pandemic, and proved to not be scalable across the city with the resources available, thus potentially causing an inequity in the offer to care home residents across Sheffield.
Therefore, following a thorough options appraisal, the Ageing Well Collaborative Group have agreed to transition to implementation of the RESTORE2mini model from April 2023.
