The Team Around the Person (TAP) model aims to support the integration of health (physical and mental) and social care. The process focuses on preventing problems escalating, supporting people to be independent at all stages of life, coordinating care across health and social and improve joint working. Roll-out of the model will be aligned with Sheffield City Council Localities boundaries, working closely with the local Primary Care Networks, and making links to the newly formed Local Area Committees as they develop their role in their communities.
TAP’s vision is to place the individual at the centre of their care and support, ensuring that their voice is heard and to help them shape the outcomes that they want to achieve. It is important that the individual receives the right care at the right time.
Our aims for the Team Around the Person approach is to:
- ensure individuals are placed at the centre of their care
- deliver better health and social care outcomes for individuals
- delay and reduce the need for care and support (where appropriate)
- deliver services that meet the needs of the individual quicker and closer to home
- improve the experience of care for individuals and carers,
- improve joint working and co-ordination of multi-agency provision,
- reduce health and social care costs

The following updates were given in April 2023:
- Since 2019, TAP received 722 referrals.
- On a cross section of closed cases:
- 98% of cases referred were individuals with 2 or more long term health conditions (including mental health and substance misuse)
- 41% of cases referred were for those with Frailty
- 58% of cases were for individuals living in areas of deprivation
- TAP has lead to an estimated 4000 less police call outs, reduced Emergency Department visits by 847, saved 3069 hospital bed days, and avoided 1470 ambulance call outs.
- On a cross section of closed cases:
- Team Around the Person offering a citywide funded model, with applications to the MJ and Health Service Journal Awards in progress in recognition of this unique collaborative offer. TAP and the Ageing Well team have also been asked to present at the NHS ConfedEXPO in June 2023.
