The HCP uses public and staff feedback as well as health and care outcomes information to identify where there is a need for a change and improvement to how people are supported in the city. HCP delivery areas are responsible for engaging with the public and relevant staff groups to develop recommendations and improvement plans. These go to the Sheffield Health and Care Partnership Board for approval. In some cases, decisions will need to be agreed upon by each of the individual HCP partner organisation boards.
