The Sheffield HCP or Health and Care Partnership is a partnership between seven health, social care and voluntary sector organisations across the city of Sheffield that work together to coordinate and deliver care.
These are Primary Care Sheffield Ltd, Sheffield Children’s NHS Foundation Trust, Sheffield City Council, Sheffield Health and Social Care NHS Foundation Trust, Sheffield Teaching Hospitals NHS Foundation Trust, NHS South Yorkshire Integrated Care Board, Voluntary Action Sheffield.
