Hear about the work of AWARE-IBD, a patient-led service improvement programme carried out by The Inflammatory Bowel Disease Centre at Sheffield Teaching Hospitals and the University of Sheffield, working with Crohn’s and Colitis UK and local partners in Sheffield, working to re-design services and deliver improved outcomes for those living with Inflammatory Bowel Disease (ulcerative colitis and Crohn’s).
The study aims to put patients in control of their care by: i) co-designing a structure to tell teams what is important to them; and ii) making that visible in multidisciplinary care team meetings and patient consultations to optimise care.
There will also be time to ask questions to the presenters and consider how we continue putting people at the heart of what we do in all our practices.
