The Sheffield HCP comprises seven formal partner organisations in the city who work together to focus on issues that can only be addressed together:
- Sheffield Children’s NHS Foundation Trust
- Sheffield City Council
- Sheffield Health and Social Care NHS Foundation Trust
- Sheffield Teaching Hospitals NHS Foundation Trust
- NHS South Yorkshire Integrated Care Board
- Primary Care Sheffield Ltd
- Voluntary Action Sheffield
The Sheffield HCP also works with Healthwatch Sheffield to engage with service users and the public to co-design, deliver and transform the Sheffield care system.
